If you have been awarded a FAILSAFE grant and wish to submit a change request, kindly complete the form below. Once we receive your submission, the request will be shared with the FAILSAFE Management Board for consideration. They may require additional information in support of the request. If a request is approved, you will receive official documentation to confirm this change from our legal team.
